One $5 Starbucks Card per household per valid email address.  Final discretion of one $5 Starbucks Card delivery determined by GameTech, International.  Only valid GameTech customers, both current and former, and/or legitimate GameTech prospects, will be fulfilled with one $5 Starbucks Card upon survey submission.
